FinishoffvocabularyexercisesinPart4.Finishoffgrammarexercises(pastparticiples)inPart6.WriteashortpassagetopresentyourideasaboutEQorIQ.Homework THANKYOU 人工剥离胎盘术 人工剥离胎盘术是用手剥离，取出滞留于宫腔内胎盘的手术。 【适应证】1．胎儿娩出后30min，胎盘仍未剥离排出者。2．胎儿娩出后，经按摩宫底或用宫缩剂等处理，胎盘不能完全排出者。3.胎儿娩出后，胎盘部分剥离引起子宫大量出血。 【禁忌证】植入性胎盘，切勿强行剥离。 【麻醉】一般不需要麻醉，可适量给予镇静剂。子宫颈内口较紧时，可肌注哌替啶50-100mg，也可用全身麻醉。 【手术步骤】1.产妇取膀胱截石位，术者更换手术衣及手套，外阴重新消毒，换消毒巾，导尿。2.术者一手放在腹部宫底部向下推压宫体，另一手手指并拢呈圆锥形，循脐带伸入宫腔，找到胎盘边缘，如胎盘为已剥离但被宫颈嵌顿者，可将胎盘握住，顺一个方向，旋转取出。若胎盘尚未剥离，术者手背紧贴宫壁，掌面向胎盘的母体面，以手掌的尺侧缘慢慢将胎盘自宫壁分离，等全部胎盘剥离后始可握住全部胎盘，在宫缩时用手牵引脐带协助胎盘娩出。切忌抓住部分胎盘，人为造成胎盘破碎，出血多。 术后肌注宫缩剂，立即检查胎盘胎膜是否完整。如有缺损，应重新伸手入宫腔，取出残留物，如手取不净时，可用大钝头刮匙刮宫。 【注意事项】1．注意产妇一般情况，术前备血，如失血多，应迅速输血。2．操作要轻柔；切忌强行剥离或用手抓挖宫腔，以免损伤子宫。3.剥离时发现胎盘与子宫壁之间界限不清，找不到疏松的剥离面不能分离者，应疑为植入性胎盘，不可强行剥离。当产后出血不能控制时，应行子宫切除术。4．术后检查胎盘，疑有胎膜胎盘残留时，应再次以手伸入宫腔，取出残留的组织或用干纱布擦拭宫腔，必要时行刮宫术。5.应尽量减少宫腔内操作次数，术后用抗生素预防感染。'
