FinishoffvocabularyexercisesinPart4.Finishoffgrammarexercises(pastparticiples)inPart6.WriteashortpassagetopresentyourideasaboutEQorIQ.Homework THANKYOU 人工剥离胎盘术 人工剥离胎盘术是用手剥离,取出滞留于宫腔内胎盘的手术。 【适应证】1.胎儿娩出后30min,胎盘仍未剥离排出者。2.胎儿娩出后,经按摩宫底或用宫缩剂等处理,胎盘不能完全排出者。3.胎儿娩出后,胎盘部分剥离引起子宫大量出血。 【禁忌证】植入性胎盘,切勿强行剥离。 【麻醉】一般不需要麻醉,可适量给予镇静剂。子宫颈内口较紧时,可肌注哌替啶50-100mg,也可用全身麻醉。 【手术步骤】1.产妇取膀胱截石位,术者更换手术衣及手套,外阴重新消毒,换消毒巾,导尿。2.术者一手放在腹部宫底部向下推压宫体,另一手手指并拢呈圆锥形,循脐带伸入宫腔,找到胎盘边缘,如胎盘为已剥离但被宫颈嵌顿者,可将胎盘握住,顺一个方向,旋转取出。若胎盘尚未剥离,术者手背紧贴宫壁,掌面向胎盘的母体面,以手掌的尺侧缘慢慢将胎盘自宫壁分离,等全部胎盘剥离后始可握住全部胎盘,在宫缩时用手牵引脐带协助胎盘娩出。切忌抓住部分胎盘,人为造成胎盘破碎,出血多。 术后肌注宫缩剂,立即检查胎盘胎膜是否完整。如有缺损,应重新伸手入宫腔,取出残留物,如手取不净时,可用大钝头刮匙刮宫。 【注意事项】1.注意产妇一般情况,术前备血,如失血多,应迅速输血。2.操作要轻柔;切忌强行剥离或用手抓挖宫腔,以免损伤子宫。3.剥离时发现胎盘与子宫壁之间界限不清,找不到疏松的剥离面不能分离者,应疑为植入性胎盘,不可强行剥离。当产后出血不能控制时,应行子宫切除术。4.术后检查胎盘,疑有胎膜胎盘残留时,应再次以手伸入宫腔,取出残留的组织或用干纱布擦拭宫腔,必要时行刮宫术。5.应尽量减少宫腔内操作次数,术后用抗生素预防感染。'
